Functional Lab Order Form

Please complete this form to request functional lab work (including GI MAP, DUTCH, or blood work) through Emmalee’s Evexia practitioner account. To order HTMA, please complete this form instead.

Regarding blood work, please read - blood draws cannot be completed within NY, NJ, or RI. Clients in these areas will either have to travel to an out-of-state Lab Corp draw site, or use a direct-to-consumer lab ordering service, such as LabCorp on Demand, in which the client will order a panel themselves and you will interpret for them.

Tests are ordered at lab cost only — no markup — plus a flat $10 fee to cover shipping for the kit. Shipping fee not required for blood orders. If ordering both GI MAP and DUTCH at the same time, only one $10 shipping fee is required as kits will ship together.

Please note: Ordering through this form does not include interpretation, consulting, or protocol support. If you'd like guidance on using the results, you’ll need to interpret them independently or book a separate consult.

Once you submit the form, I’ll confirm your order via email. If payment has not yet been received, the total owed will be included in the email. Turnaround time for shipping kits is usually within 2 business days.

Kits will not be shipped until payment is received.
